Rundown: The COVID-19 vaccination process

The Department of Health (DOH) has issued a recommended vaccination process flow on-site. It noted that local government units (LGUs) may tweak or change the following process based on their needs and convenience.

1st Dose

Pre-vaccination process

Step 1: Prior to your registration, determine which priority group you belong to. Click here.

Step 2: Pre-vaccination screening. Only persons with any of the following conditions will be required a medical clearance:

• Autoimmune disease
• HIV
• Cancer or Malignancy
• Transplant Patient
• Steroid Treatment
• Terminal illness, bed ridden

Step 3: Register through your local government unit (LGU). Fill out the registration form (online or offline). You can also consult your barangay, Barangay Health Workers (BHWs), Barangay Health Emergency Response Teams (BHERTs), LGUs, Public Information Office, or hotlines for further instructions.

Step 4: Wait for your vaccination schedule. No walk-ins allowed! 

Step 5: Prepare the required documents such as:

• Any government ID
• Any proof of comorbidity, if needed, any one of the following:
  • Prescription
  • Medical Certificate
  • Other hospital records

Vaccination day

On your vaccination day, your body temperature will be checked upon entry.

Step 1: Confirm your registration by presenting a valid government ID.

Step 2: Read the printed materials given to you. These include a vaccine fact sheet, a health screening form, and an informed consent form.

Step 3: Clarify doubts and ask questions before signing the informed consent form.

Step 4: Health screening. Disclose your health history.

Step 5: Get vaccinated. After which, you will be given your vaccination card with the date of your 2nd dose.

Step 6: You will be observed at the vaccination site for 15 to 30 minutes before you can be sent home.

After vaccination

If you feel any pain or uncomfort feeling, consult a doctor right away or call the LGU, vaccination site’s hotline, or the nearest health facility.

2nd Dose

Pre-vaccination process

Your second dose of COVID-19 vaccine is as important as your first dose. Thus, set a reminder for the schedule of your 2nd dose inoculation.

Vaccination day

Your body temperature will be checked upon entry to the vaccination site.

Step 1: Present your vaccination card and a valid government ID.

Step 2: Health screening. Disclose your complete health history.

Step 3: Clarify doubts and ask questions.

Step 4: Take the second shot!

Step 5: You will be observed at the vaccination site for 15 to 30 minutes.

Before leaving the jab site, make sure that your vaccination card is filled correctly. It should be marked on your card that you received your second dose of the vaccine, and take note of the health facility or hotline number to report in case you experience any adverse events following the immunization.

You will then be asked to go straight home and self-monitor. If any symptoms persist or worsen, consult a doctor right away or call the LGU, vaccination site hotline, or the nearest health facility.
